The name of the book is “When Panic Attacks” by Dr David Burns. The book is full of examples and is set up like a workbook so you can enter your own comments. I strongly recommend it. It may be a bit hard to find because there is only one printing of it, but if you search online, you can find it. It’s also available in most libraries if you just want to browse through it.
Dr. Burns also wrote a book that focuses more on depression than anxiety. It’s called “Feeling Good, The New Mood Therapy.”
